For the more than 400 million people worldwide living with G6PD (glucose-6-phosphate dehydrogenase) deficiency — a significant proportion of whom reside in the United States — medication safety is not an abstract concern. It is an everyday reality. The wrong drug, prescribed without knowledge of a patient's G6PD status, can trigger acute hemolytic anemia: a sudden, potentially life-threatening destruction of red blood cells.

Yet despite the well-documented relationship between G6PD deficiency and drug-induced hemolysis, many patients report that their condition is not consistently noted in medical records, not communicated across care settings, and not considered when prescriptions are written. This article provides a detailed, category-by-category overview of medications relevant to G6PD patients — including those that are contraindicated, those that require caution, and those that are generally considered safe — to help patients advocate for themselves at every point of care.

Why Certain Drugs Trigger Hemolysis in G6PD Patients

G6PD is an enzyme that plays a critical role in protecting red blood cells from oxidative damage. When this enzyme is deficient or absent, red blood cells become highly vulnerable to oxidative stress. Certain medications generate reactive oxygen species as part of their metabolic activity. In patients with normal G6PD function, these reactive molecules are neutralized efficiently. In G6PD-deficient patients, they accumulate and destroy red blood cell membranes, leading to hemolysis.

Medications That Are Contraindicated or High Risk

Antimalarials

Primaquine and tafenoquine are the two antimalarial drugs most strongly associated with severe hemolysis in G6PD-deficient individuals. Both are 8-aminoquinoline class drugs and are used to treat or prevent relapsing malaria caused by Plasmodium vivax. The FDA-approved prescribing information for tafenoquine (Krintafel) explicitly requires G6PD testing before administration. Chloroquine and hydroxychloroquine carry a lower but still documented risk and should be used with caution.

Nitrofurantoin

Nitrofurantoin, a common antibiotic used to treat urinary tract infections in the United States, is considered high risk for G6PD patients. It is frequently prescribed in outpatient settings, often without G6PD status being considered. Patients who are G6PD deficient and prescribed nitrofurantoin for a UTI should alert their prescribing provider and request an alternative antibiotic.

Dapsone

Dapsone, used to treat leprosy, dermatitis herpetiformis, and certain infections in immunocompromised patients, carries significant hemolytic risk for G6PD-deficient individuals. It is also found in some topical acne medications, where systemic absorption may still pose a risk in severely deficient patients.

Rasburicase

Rasburicase, used to manage elevated uric acid levels in cancer patients undergoing chemotherapy, is absolutely contraindicated in G6PD deficiency. It generates hydrogen peroxide as a byproduct of its mechanism of action and can cause severe, rapid hemolysis. G6PD testing is required before rasburicase administration.

Methylene Blue

Methylene blue, used to treat methemoglobinemia, is paradoxically ineffective and potentially harmful in G6PD-deficient patients. Its mechanism of action depends on G6PD activity; in deficient patients, it fails to work and may worsen oxidative stress.

Medications That Require Caution and Monitoring

Aspirin and NSAIDs

High-dose aspirin has historically been listed as a risk factor for G6PD-related hemolysis, though evidence suggests that standard low-dose aspirin (81 mg daily, commonly used for cardiovascular protection) is generally tolerated by most G6PD patients. High-dose aspirin should be used with caution. Other NSAIDs such as ibuprofen and naproxen are generally considered lower risk but warrant monitoring in patients with severe deficiency variants.

Sulfonamide Antibiotics

Sulfonamide-class antibiotics — including sulfamethoxazole, commonly found in the combination antibiotic trimethoprim-sulfamethoxazole (Bactrim) — are associated with hemolysis risk in G6PD patients. This combination antibiotic is widely prescribed in the US for UTIs, ear infections, and other bacterial infections. Patients should ensure their G6PD status is documented before receiving this medication.

Quinolone Antibiotics

Fluoroquinolones such as ciprofloxacin and levofloxacin are frequently prescribed antibiotics that carry a moderate hemolysis risk for G6PD patients. They are not absolutely contraindicated, but patients with severe G6PD deficiency should discuss alternatives with their provider when possible.

Vitamin K (High Dose)

High-dose synthetic vitamin K (menadione) has been associated with hemolysis in G6PD-deficient newborns. Standard vitamin K injections given at birth in the United States use phytonadione, which is generally considered safe.

Knowledge alone is not sufficient protection. G6PD patients in the United States are encouraged to take the following practical steps:

  1. Ensure your G6PD status is documented in your electronic health record at every care facility you use, including your primary care provider, specialists, urgent care centers, and any hospital where you might receive emergency care.
  2. Carry a medical alert card or wear a medical alert bracelet indicating your G6PD deficiency status.
  3. Use a single pharmacy whenever possible so that pharmacists can cross-reference your medication history and flag potential interactions or contraindications.
  4. Ask specifically about G6PD safety whenever a new medication is prescribed, even if you have been seeing the same provider for years.
  5. Consult the G6PD Deficiency Association's drug list and other credible resources when evaluating any new prescription, over-the-counter drug, or supplement.

A Note on Severity and Individual Variation

It is important to recognize that G6PD deficiency is not a single, uniform condition. There are hundreds of known G6PD variants, classified by the World Health Organization into severity classes. A patient with a Class I variant (the most severe) faces substantially greater risk from borderline medications than a patient with a Class III variant. Discussing your specific variant and enzyme activity level with a hematologist or genetic counselor can help you make more nuanced, personalized decisions about medication safety.
