Yet for a striking number of patients whose family histories and ethnic backgrounds place them squarely within high-risk demographic groups, a diagnostic blood test is never ordered. No referral is made. No conversation is initiated.

The result is a quiet public health failure, one that unfolds not in dramatic, headline-generating moments, but in the accumulated weight of delayed diagnoses, preventable hemolytic crises, and patients who spend years—sometimes decades—searching for explanations that a single enzyme assay could have provided.

Who Is Most Affected—and Why That Matters

G6PD deficiency follows a geographic and ancestral pattern that is well-documented in the scientific literature. Individuals with ancestry from sub-Saharan Africa, the Mediterranean basin (including Greece, Italy, and Turkey), the Middle East, South and Southeast Asia, and certain Pacific Island communities carry measurably higher rates of G6PD-deficient gene variants. This distribution is not coincidental. The enzyme deficiency appears to confer partial resistance to malaria, meaning it persisted in populations historically exposed to the parasite—precisely those regions where malaria has been endemic for millennia.

This is foundational knowledge in hematology and genetics. And yet, in routine clinical encounters across the United States, that knowledge often fails to translate into action. Providers may not ask about family heritage. Patients may not volunteer it, or may not know their ancestry in sufficient detail. Insurance coverage for G6PD testing remains inconsistent, and in many primary care settings, the test is simply not part of any standard screening protocol for adults.

The consequence is that patients who most need to know their G6PD status are frequently the last to find out.

Second, the condition's variable expressivity complicates recognition. Many G6PD-deficient individuals remain entirely asymptomatic until exposed to an oxidative stressor—a specific antibiotic, an antimalarial drug, fava beans, or a significant infection. Without a triggering event, the deficiency leaves no obvious clinical footprint, making it easy for providers to overlook during routine wellness visits.

Third, and perhaps most significantly, implicit racial and ethnic bias in clinical decision-making continues to shape which patients receive thorough diagnostic workups and which do not. Research has consistently demonstrated that patients of color in the United States receive less thorough diagnostic evaluations across a range of conditions. G6PD deficiency, which disproportionately affects Black and Brown patients, is not immune to this pattern.

Insurance Coverage and the Economics of Exclusion

Beyond clinical culture, economic structures further complicate access to testing. G6PD enzyme assays are not universally covered by insurance plans, and coverage criteria vary considerably across payers. In some cases, a test may be covered only when ordered in conjunction with a specific diagnosis code—meaning a patient who has not yet been diagnosed cannot easily obtain the test that would establish the diagnosis in the first place. This circular logic creates a bureaucratic barrier that falls hardest on patients who are already navigating under-resourced healthcare environments.

For uninsured or underinsured patients, the out-of-pocket cost of a G6PD enzyme quantitative assay can range from approximately $30 to over $150 depending on the laboratory and geographic location—a meaningful expense for households with limited discretionary income, and one that providers rarely discuss proactively.

Medicaid coverage for G6PD testing also varies by state, adding a geographic dimension to an already uneven landscape. Patients in states with more restrictive Medicaid programs may face greater obstacles to testing than those in states with broader coverage provisions.

What Newborn Screening Does—and Doesn't—Cover

Some states include G6PD deficiency in their newborn screening panels, but national coverage is far from uniform. As of the most recent review of state-level screening programs, only a minority of states mandate G6PD testing at birth. This means that a child born in one state may be routinely screened, while an identical child born in a neighboring state receives no such evaluation—a disparity that has no basis in clinical logic and reflects instead the patchwork nature of public health policy in the United States.

For adults who were born before G6PD screening was available in their state, or who were born in countries where newborn screening programs are less comprehensive, the window for early identification has already passed. These individuals must rely on clinical recognition and provider initiative—resources that, as this article has established, are unevenly distributed.

Advocating for Yourself: Concrete Steps Toward a Diagnosis

Understanding the systemic barriers to G6PD testing is not merely an academic exercise. It is the foundation for effective self-advocacy. The Broader Stakes

The testing gap surrounding G6PD deficiency is not a peripheral concern. It sits at the intersection of medical education, institutional bias, insurance policy, and public health infrastructure—forces that together determine which patients are seen, which conditions are recognized, and which communities bear the burden of preventable harm. For patients of African, Mediterranean, Middle Eastern, and Southeast Asian descent, this gap represents a concrete, quantifiable health risk.

Knowledge of one's G6PD status is not a luxury. It is a clinical necessity that shapes every medication decision, every dietary choice, and every conversation with a healthcare provider. The first step toward closing this gap is understanding that it exists—and that patients themselves, armed with the right information, are among the most powerful forces available to bridge it.
