For most women, preparing for pregnancy means prenatal vitamins, lifestyle adjustments, and conversations with an OB-GYN. For women living with glucose-6-phosphate dehydrogenase (G6PD) deficiency, however, that checklist carries an additional layer of complexity — one that is frequently underestimated, misunderstood, or missed entirely by the healthcare providers they trust most.

G6PD deficiency is the world's most common inherited enzyme disorder, affecting an estimated 400 million people globally. In the United States, it disproportionately impacts individuals of African, Mediterranean, Middle Eastern, and Southeast Asian descent. While the condition is often discussed in the context of medication reactions or dietary triggers, its implications for reproductive health remain a largely underdiscussed chapter of the G6PD story.

That silence has real consequences — for mothers, and for the newborns who may inherit the gene.

Understanding the Genetics Before You Conceive

G6PD deficiency is an X-linked recessive disorder, which means the gene responsible for producing the G6PD enzyme is located on the X chromosome. This inheritance pattern creates a reproductive dynamic that every woman with the condition — or who carries the gene — should understand before conception.

Biological males, who carry only one X chromosome, will express the disorder if that chromosome carries the deficient gene. Biological females, with two X chromosomes, may be full carriers, mildly affected, or, in cases where both X chromosomes carry the variant, significantly affected. A woman with G6PD deficiency who conceives with a partner who also carries the gene faces a meaningful probability of having a daughter who is fully affected — not merely a carrier.

Genetic counseling before conception is not a luxury for G6PD-positive women; it is a practical tool for informed family planning. A genetic counselor can map out inheritance probabilities, explain the spectrum of disease severity, and help prospective parents think through their options with clarity rather than anxiety.

"Many of my patients are surprised to learn that daughters can be fully affected," noted one maternal-fetal medicine specialist practicing in the Southeast. "There's a widespread assumption that G6PD is a 'male disease.' Correcting that misconception early in the conversation changes everything about how families prepare."

Medication Safety During Pregnancy: A Minefield Worth Mapping

Pregnancy introduces a host of medications, supplements, and interventions — many of which have never been adequately studied in G6PD-deficient populations. The challenge is compounded by the fact that oxidative stress, which triggers hemolytic crises in G6PD-deficient individuals, can be provoked by substances that are otherwise considered safe or even beneficial in pregnancy.

Iron supplementation, routinely prescribed to prevent anemia during pregnancy, is generally well-tolerated by G6PD-deficient women at standard doses. However, high-dose iron therapy, sometimes used to treat severe gestational anemia, has been associated with oxidative stress and should be approached with caution and close monitoring.

Vitamin K administration, given to newborns immediately after birth to prevent hemorrhagic disease, has historically been flagged in the context of G6PD deficiency. Current clinical guidance supports the use of intramuscular vitamin K in newborns, including those with G6PD deficiency, as the risk of not administering it far outweighs the minimal hemolytic risk. However, large oral doses of synthetic vitamin K analogs (menadione) remain contraindicated.

Antibiotic choices during labor and delivery also warrant scrutiny. Nitrofurantoin, commonly prescribed for urinary tract infections during pregnancy, is classified as potentially harmful for G6PD-deficient individuals and should be avoided, particularly near term. Women should proactively inform every obstetric provider — from their midwife to the anesthesiologist — of their G6PD status before any medication is administered.

Creating a written medication safety card, updated with current G6PD risk classifications from resources such as the World Health Organization's revised 2022 classification system, is a practical step that many patient advocates recommend.

The Newborn Screening Gap

One of the most urgent reproductive health concerns for G6PD-positive mothers is what happens after delivery. Newborn jaundice — hyperbilirubinemia — is significantly more common and potentially more severe in infants with G6PD deficiency. Without prompt identification, severe neonatal jaundice can progress to kernicterus, a form of brain damage that is entirely preventable with timely intervention.

Despite this well-established risk, G6PD deficiency is not included in the standard Recommended Uniform Screening Panel (RUSP) that guides newborn screening programs across the United States. As of this writing, only a small number of states include G6PD in their routine newborn metabolic screening panels. The result is a geographic lottery: a baby born in Pennsylvania may be screened; a baby born in neighboring Ohio may not be.

For G6PD-positive mothers, this gap demands proactive advocacy. Before delivery, women should:

"The parents who come in knowing their status and asking the right questions are the ones whose babies get the monitoring they need," said one neonatal nurse practitioner at a large urban hospital in the Midwest. "But it shouldn't be that way. The system should catch every child, regardless of whether the mother already knows."

Breastfeeding, Diet, and the Postpartum Period

Breastfeeding is encouraged for G6PD-deficient mothers, with one important caveat: certain substances consumed by the nursing mother can be transferred through breast milk to the infant. Fava beans, a classic G6PD trigger, should be avoided by breastfeeding mothers of G6PD-deficient infants. Similarly, some herbal supplements marketed for lactation support — including certain preparations containing naphthalene compounds or oxidative agents — may pose indirect risks.

The postpartum period also brings its own medication considerations. Some analgesics, including aspirin at high doses, and certain antimalarials prescribed in regions where malaria prophylaxis is relevant, carry G6PD risk classifications that nursing mothers should review carefully with their providers.

Mental health is another dimension of postpartum care that intersects with G6PD awareness. The anxiety of managing a genetic condition, monitoring a newborn for jaundice, and navigating a healthcare system that frequently lacks G6PD literacy can be isolating. Patient communities, including online forums and advocacy organizations, offer a form of peer support that clinical care alone often cannot provide.

Advocating for Yourself in an Undertrained System

The most consistent theme emerging from conversations with G6PD-positive mothers across the United States is the burden of self-education. Time and again, women describe arriving at obstetric appointments armed with printed references, only to find providers who are unfamiliar with the condition's reproductive implications.

This reality is not an indictment of individual clinicians — G6PD deficiency receives limited attention in standard medical and nursing curricula. It is, however, a call to action for patients, advocacy organizations, and healthcare institutions alike.

G6PD-positive women entering pregnancy deserve a care team that understands the condition's inheritance patterns, recognizes its medication safety implications, and knows how to advocate for newborn screening on their behalf. Where that expertise is absent, patients must be equipped to fill the gap — not because the burden should fall on them, but because, right now, it often does.

Knowledge, in this context, is not merely empowering. For a G6PD-positive mother and her newborn, it can be lifesaving.
